- Mailing address: Click “Same as permanent address”
- Taxpayer Identification Number (TIN):
- Base on the tax form we see, it is tax Identification Number or something else, we will choose the one that is the same with tax form when renew.
Then fill in the basic information and finish is done. Signature is the name of account.
